Case Attempt: A.C. is a 17-y.o. female who presents with complaints of increased fatigue/ tiredness x 2 months. She endorses feelings of frustration, irritability, and mood lability; associated anhedonia, poor school performance, and unintentional weight gain (approx. 25-30 lbs.). States symptoms were initially triggered by a breakup with her boyfriend but have increased recently. Endorses frequent episodes of crying and hopelessness. She demonstrates poor eye contact and is noted to be tearful during the visit. Exam reveals an obese young woman with abdominal striae, acanthosis nigricans, and mild acne on forehead. She denies suicidal and homicidal ideations and denies thoughts of self-harm. Social history notable for marijuana and alcohol use. She had been sexually active with her boyfriend but denied having multiple partners or recent activity.

Associated symptoms include frustration, irritability, anhedonia, decreased school performance, emotional lability, and unintentional 25–30 lb weight gain. Physical exam notable for obesity, abdominal striae, acanthosis nigricans. No significant past medical history. Social history includes sexual activity, alcohol and marijuana use. She denies suicidal ideation or self-harm, but reports wanting to “disappear.”
Case Attempt: AC is a 17 y/o F established pt presents with c/o fatigue and lack of energy for the last 2 months. AC also reports being easily irritated, crying more often, and a decline in her school grades and a lack of interest in once pleasurable activities. AC denies any significant medical history and reports occasionally taking Ibuprofen for menstrual cramps. PE reveals a tearful adolescent with an elevated BMI, acne on her face, striae on her abdomen, and acanthosis nigricans on her neck. AC has no significant personal hx but a family hx of DM2, obesity, and HTN.
